Maintenance always costs money - it is a shame no one wants to pay for it until someone (or somebodies) die.
Despicable scenario.
While I don't think something like this is maliciously intended or orchestrated, it is certainly the outcome of decisions to either a) not have maintenance, b) delay it, or c) not even realise it was required.

That's true. I don't think all such events are orchestrated, but I do think it pays to be skeptical generally.
